May 2, 2016

Dear Patients:

It is with mixed emotions that I am announcing my retirement from active practice, effective June 17, 2016. It has been a great pleasure providing for your health care needs over the years, but it is time for me to move on.

As of June 17, 2016, Dr. Robert Raines will be taking over my patients. I am pleased that you have the opportunity to have him as your physician. Dr. Raines has been my partner at Lone Star Orthopaedics for over 16 years.  Our practice patterns and personal demeanor are very similar.   I am glad to have left my patients in his capable hands. Of course, you may seek medical care from another doctor if you like.  If you choose to do so, I recommend looking for a fellowship-trained, foot and ankle orthopaedic specialist as soon as possible.

If you would like to continue to receive care at Lone Star Orthopaedics, no action on your part is needed.  However, if you would like to transfer your care elsewhere, we can assist you. Your medical records are confidential, and a copy can be transferred to another doctor/clinic or released to you or another person you designate only with your permission. If you wish to have your medical records transferred, please send a medical authorization form to the office as soon as possible, so we may transfer your records to your new doctor.  Please include your new doctor’s name and address.  Unless you request a transfer of your medical records, they will remain on file at this office.

I have greatly valued our relationship, and thank you for your loyalty and friendship over the years. Best wishes for your future health.

 Sincerely,

James A. Amis, M.D.
